Welcome to Barnard! Before you arrive on campus, you'll need to complete several health related forms, all due by December 9, 2013. These forms are located on the Primary Care Health Service Open Communicator website. Please note that while all forms are located on the Open Communicator website, some will be submitted online and some must be printed out and faxed or mailed to Primary Care.
In order to complete the necessary forms, you will need to schedule an appointment with your health care provider (either a nurse practitioner, physician, or physician's assistant) to fill out and sign the Incoming Student Immunization From (found on the Open Communicator website), and to get any required vaccinations. We suggest making an appointment with your health care provider ASAP, to ensure that you have ample time.
Please note that New York State law requires that all students have documentation of 2 measles, mumps and rubella (MMR) vaccinations or individual measles/mumps/rubella immunizations or serologic (blood test) evidence of immunity and that all students complete the “Meningococcal Meningitis Vaccination Response Form” (found on the Open Communicator website). These forms are REQUIRED to be permitted on campus and to attend Barnard.

All VISP students are required to purchase the Barnard College Aetna Student Health Insurance. Below you will find a comparison chart to help you familiarize yourself with the plan.
